Appendix F - Decision Latency Register
This appendix provides the four decision latency tracking logs referenced in Chapter 21, plus the quarterly summary template used to produce the board-level governance health report. The ACG owns the register. The CTO reviews the quarterly summary. The board receives the summary as part of the governance health report.
Log 1 — Authorization Decisions
One entry per executor deployment submitted for ACG review. Filed by the ACG lead at submission and updated at completion.
Latency computed: Completion date minus submission date, in calendar days.
Metrics produced: Rolling 4-week average by review type. Trend vs prior quarter.
Signal thresholds: Average latency more than 50% above target for two consecutive months. Any single review exceeding three times the average.

Log 2 — Delegation Decisions
One entry per authority grant or authority withdrawal decision. Filed by the ACG lead when the authority grant record from Appendix D is received.
Metrics produced: Count of decisions per quarter by execution mode. Days from initial request to decision date.
Signal thresholds: Zero fully automated or system-led grants in any quarter where the authorization log shows new executors in those modes. Any grant decision completed in under 3 business days for fully automated mode.

Log 3 — Recalibration Decisions
One entry per recalibration decision completed. Cross-referenced against the ownership register to identify executors with no recalibration activity.
Metrics produced: Count of executors with no recalibration in past 12 months as percentage of total. Ratio of signal-triggered to cadence-triggered recalibrations.
Signal thresholds: More than 20% of consequential executors without recalibration in 12 months. Cadence-triggered recalibrations exceeding signal-triggered for two consecutive quarters.

Log 4 — Model-Level Decisions
One entry per model-level decision. Maintained by the CTO and filed in the ACG governance library as the authoritative record of strategic execution surface decisions.
Metrics produced: Days since last model-level decision. Whether the next scheduled review date has been met.
Signal thresholds: More than 12 months since last entry. Any missed scheduled review date without a rescheduled date recorded within 30 days.

Quarterly Summary — Board Report Input
Complete at the end of each quarter using data from the four logs. This is the source for the board-level governance health report described in Chapter 21. The CTO completes and presents this summary — it is not delegated to the ACG.
