Appendix D - Authority Decision Templates
This appendix provides the two operational templates referenced in Chapter 19: When the System Decides. Together they form the authority decision record — the documentation that distinguishes governed delegation from automation at scale.
Both templates should be filed in the ACG governance library. The authority grant record is filed before deployment. The authority withdrawal record is filed alongside the original grant record when authority is reduced or removed, and must be accompanied by a new grant record if the executor is being revised to a lower execution mode rather than fully withdrawn.
Template 1 — Authority Grant Decision
Complete for every new executor entering the execution surface in human-assisted, system-led, or fully automated mode. File before deployment. This record constitutes the authorization basis for the executor's authority on the execution surface.
| FIELD | GUIDANCE | ORGANIZATION ENTRY |
|---|---|---|
| Executor name | Name and version of the system being authorized | [to be completed] |
| Executor type | Automation / AI agent / vendor execution dependency | [to be completed] |
| Authority scope | The class of decisions this system is authorized to make — in organizational consequence terms, not technical function description | [to be completed] |
| Execution mode | Human-assisted / System-led / Fully automated | [to be completed] |
| Scope boundaries | Explicit cases or conditions outside the authorized scope — what this system is not authorized to decide without human review | [to be completed] |
| Governance owner | Named person accountable for ongoing fitness of this delegation | [to be completed] |
| Escalation owner | Named person holding authority for cases the system defers (system-led mode only; leave blank for fully automated) | [to be completed] |
| Monitoring model | How behavioral drift will be detected — metrics tracked, frequency, who is alerted, threshold for intervention (fully automated mode) | [to be completed] |
| Withdrawal conditions | Circumstances triggering a full authority review rather than a recalibration — market shifts, regulatory changes, performance thresholds | [to be completed] |
| Decision maker | Named person accepting accountability for this authorization decision | [to be completed] |
| Decision date | Date of authority grant decision | [to be completed] |
| ACG review ref. | Reference number or date of ACG deployment review confirming governance fitness | [to be completed] |
Template 2 — Authority Withdrawal Decision
Complete when reducing or removing a system's authorized execution mode or authority scope. File alongside the original authority grant record. A withdrawal that changes the execution mode requires both this record and a new authority grant record at the revised scope and mode.
| FIELD | GUIDANCE | ORGANIZATION ENTRY |
|---|---|---|
| Executor name | Name and version of the system being revised | [to be completed] |
| Original auth. ref. | Reference to the original authority grant record (date or record number) | [to be completed] |
| Withdrawal trigger | Recalibration cascade / Withdrawal condition met / Scope assessed as too broad | [to be completed] |
| Current scope | Authority scope as currently authorized per the grant record | [to be completed] |
| Revised scope | Authority scope after withdrawal — or 'Fully withdrawn' if removing from execution surface | [to be completed] |
| Current mode | Current authorized execution mode | [to be completed] |
| Revised mode | Revised execution mode after withdrawal — e.g. Fully automated → System-led | [to be completed] |
| Reason | Why the original authorization is no longer appropriate — in organizational consequence terms | [to be completed] |
| Transition plan | How the change will be implemented: timeline, interim human coverage if needed, ACG review gate before revised mode goes live | [to be completed] |
| Decision maker | Named person accepting accountability for this withdrawal decision | [to be completed] |
| Decision date | Date of withdrawal decision | [to be completed] |
| New grant ref. | If revising rather than fully withdrawing: reference to the new authority grant record at revised scope and mode | [to be completed] |
